I UNDERSTAND THAT THIS IS A LEGALLY BINDING DOCUMENT AND I AM, AMONG OTHER THING, WAIVING LEGAL RIGHTS THAT MY spouse/legal guardian, MY ABOVE NAMED CHILD AND I WOULD OTHERWISE HAVE. I HAVE HAD AN OPPORTUNITY TO READ THIS DOCUMENT IN FULL.

I, the undersigned parent or participant, on behalf of myself, my spouse/legal guardian, and my above named participant do hereby RELEASE Olympus Track Club and all of it’s agents, officers, and coaches from any liability for bodily injury to my child, death of my child, and damage to personal property belonging to me and/or my child, sustained while partaking in the Olympus Track Club program. This general RELEASE includes (without limitation) injuries arising out of negligent conduct or inaction of the Olympus Track Club’s agents, officers, or coaches from any injuries sustained by my child during participation.

I, hereby indemnify Olympus Track Club and will pay to defend and hold Olympus Track Club harmless from any claim, cause of action, lawsuit, liability and expense, including attorney’s fees (collectively “Claims”) that Olympus Track Club incurs in connection with my or my child’s use of the Olympus Track Club based upon the actions of myself or my child.

My child, my spouse/legal guardian, and I are aware that there are risks involved including the participation of sporting events and I, my spouse/legal guardian and my above named child hereby assume the risks associated with myself or my child’s participation in this program.

In addition, I am aware of the degree of physical activity in which myself or my child will be partaking in and have received approval from my physician or child’s physician for such activities.

I am providing the general RELEASE on behalf of myself, my spouse/legal guardian and my above named child as a condition of and in consideration for my child’s participation in the Olympus Track Club program. I understand that the Olympus Track Club and it’s coaches are relying upon this RELEASE intending to legally bind my spouse/legal guardian, my above named participant and myself. I have this day signed this Liability Waiver Release for children under seal.
